Emissions Inventory Forms

For Calendar Year 2007

If your business is required to report emissions to APCD, please use the forms below. See form SAM 1 for general instructions.  The forms are available in PDF format and either WordPerfect 6.x-10 format or Microsoft Word 2000-2003 format. To save a document to your computer, right-click on the link and choose "Save Target As" or "Save Link As" (depending on your browser software).

With the exception of filling in the requested information, these forms must be submitted without modification.
