A Tab Q-7 form is a facility emergency response and planning document. The document is submitted to your Local Emergency Planning committee (LEPC) and after review is submitted to the State Emergency Response Commission (SERC). You must complete a Tab Q-7 if your facility has an Extremely Hazardous Substance (EHS), as characterized by the US EPA, in a quantity greater that that listed in the code of Federal Regulations (CFR). You can look in 40 CFR Part 355 for the list of TPQs (Threshold Planning Quantities) to determine if you need to submit a Tab Q-7 or you may contact your LEPC.
